To complete the booking, Dr Mary Egan will email you to ask that you complete a consent form and intake form, for the purposes of gathering contact information, emergency contact details and a brief outline of the service you are seeking. These forms are encrypted and are not shared with Squarespace.

Please let Dr Mary Egan know if you need to change any details, for example, email address etc. If the above named Psychologist seeks to use your personal data in a way that is different from the terms and conditions when you registered, you will be contacted directly and asked your permission to do so. You will be free to decline such a request. Whenever you no longer wish to receive notifications or information from this site, please contact the above named Psychologist to be removed from the database.
